Diabetes prevention/management

Join professional coaches to learn how you can prevent and manage your diabetes! Our friendly lifestyle coaches will help you learn about healthy eating habits, exercise, and common risk factors for diabetes. Take charge of your health – contact us today!

Chronic disease prevention/management

For our neighbors who live with chronic conditions like heart disease, our community health workers help you manage your symptoms and maintain your highest quality of life. For those who are at risk of chronic diseases, our team coaches you through lifestyle changes you can make to ensure your continuing good health. Our team can also connect you with self-management classes, offering evidence-based approaches to manage your symptoms.

Assistance with quitting smoking

Quitting smoking can be a challenge, but our team is here to ensure you’re not alone. Get assistance in your effort to quit smoking through our weekly support groups; take part in wellness activities like massages and Reiki; and let us refer you to housing, employment, and other resources to eliminate stress and help you quit for good.

Food and nutrition assistance

Our team of community health workers can help you put healthy food on the table. Our food shelves in Phillips and Cedar Riverside provide easy access to a wide range of fresh and healthy foods, while our community cafés in Phillips and Near North offer pre-cooked to-go meals. We also offer nutrition classes to help you learn new, healthier eating habits to support your overall wellness. Fitness classes

We organize fitness classes in North and South Minneapolis, incorporating multiple forms of physical activity to help our neighbors manage chronic conditions and maintain their best health. Many of these classes can accommodate elders or other individuals with limited physical mobility. New classes are always starting, so let us know if you’re interested!

Rent/utilities aid

If you’re having trouble paying your rent or utilities, you may be eligible for state support. Our team can help you navigate the application process, helping ensure that all individuals and families in our neighborhoods have a stable and safe place to live.

Family crisis assistance

If your family is in a crisis situation, our team can help. We provide advocacy and support for women and families experiencing domestic violence or sexual assault, helping individuals develop a safety plan, secure safe housing, and navigate the legal system. In addition, our crisis nursery offers emergency childcare for those short-term crisis situations.

Other community referrals

In addition to the services and resources provided by Pillsbury United Communities directly, our community health team is closely connected to many other organizations providing support in our communities. We can refer you to other community organizations for any needs that may be outside our direct capabilities.
